I grant MedCerts and their subsidiaries permission to use the photographs and videos of me in all their publications, including websites, without payment or any other consideration.

I understand and agree that MedCerts owns these materials. I authorize MedCerts to edit, copy, exhibit, publish, distribute or not use my story for the purposes of publicizing the work of MedCerts. In addition, I waive the right to inspect or approve the finished product, including the written or electronic copy, where my information appears.

Additionally, I waive any right to compensation arising or related to the use of the information.
